To receive your scholarship, please submit the following information using the contact form below, or emailing directly to jltaeunice@yahoo.com no later than July 31, 2026:

• Your Student ID Number
• Proof of Enrollment (a letter of acceptance is acceptable)
• The mailing address of your school's Financial Aid Office

Important: All required information must be submitted by July 31, 2026 to claim your scholarship. Unfortunately, scholarship awards cannot be claimed after this deadline.
